Rename simplify-code → review-and-simplify-changes

Browse source 
Rename the 'simplify-code' skill to 'review-and-simplify-changes'. Update README entry to use the new skill key and rename simplify-code/SKILL.md to review-and-simplify-changes/SKILL.md, updating the name field and heading to match. Aligns the filesystem path and README with the clarified skill name.
