Oliver Woodman c62e535cf0 Make HLS behind-live-window behavior consistent. 
- Propagate BehindLiveWindowException if we fall off the back
  of an HLS live stream.
- Consolidate seekPositionUs and playbackPositionUs into a
  single parameter.
